Back pain is very common. Musculoskeletal pain is the second most common complaint made to a primary care physician. One of the most common causes is degenerative disc disease in the spine. With aging small “wear and tear” injuries occur to our spinal discs. These injuries are additive and may result in arthritis, bone spurs and accelerated degradation, which can in turn lead to low back and sciatica, stiffness, weakness, numbness and even permanent nerve damage with loss of function.
Exercise, physical therapy, anti-inflammatories medications, steroid injections, and ultimately surgery have been the mainstay of medical treatment. These treatments are aimed at maximizing function and reducing pain. They don’t though address the underlying pathology and steroids, for example, may even cause it to accelerate.
Many times the surgeon may decline offering surgery because of diffuse involvement of the spine and the lack of correlation between a specific symptom and identifiable cause on imaging. Imaging doesn’t necessarily correspond to a patient’s pain level. They are often referred to a pain clinic and many end up on long term opioids.
